Five military medical treatment facilities have been selected by the Defense Health Agency to design a new person-centric health care model. The initiative aims to improve patient access and care delivery, according to a DVIDS story.
Key features include the My Military Health digital suite and SilverCloud, a self-care wellness program. The new model, featuring tools like virtual visits and Care Companion, enhances health care plan compliance and patient-provider connection.
The communities are:
- Naval Hospital Jacksonville
- Wright-Patterson Air Force Base Medical Center, 88th Medical Group, Ohio
- Martin Army Community Hospital, Fort Moore, Georgia
- Naval Medical Center Portsmouth, Virginia
- Eglin Air Force Base Hospital, 96th Medical Group, Florida
“This is a ground-breaking evolution in military health,” said Naval Hospital Jacksonville Director Capt. Sharon House. “For the first time ever, our providers were afforded the opportunity to give direct feedback as these programs were developed. Having first-hand knowledge of what would best fit our community and patients, we will be able to provide the best care in the future.”
