Army and Pentagon investigators are looking into what went wrong Wednesday night when an Army Black Hawk helicopter collided with a regional jet from Wichita, Kansas. Sixty passengers, four airline crew members and three soldiers all died in the Potomac River after the crash, officials said.

The helicopter crew – a captain, a staff sergeant and a chief warrant officer – were based at Fort Belvoir, Virginia, The Hill reported. They were performing a routine training exercise, according to DOD.

“It’s a tragedy, a horrible loss of life,” Defense Secretary Pete Hegseth said Thursday.
